Hi, Jeff. Sorry this is happening. (Did you sign up for paid support. We may be following up with you there.)

PMPro will cancel any existing membership level before checking out and setting up the new level. Users can’t have more than one level at a time right now. So that might have happened if this user had an old level.

RE the new level not taking hold: if you are using PayPal Standard, there are a number of issues coming up that are hard for us to address.

    Some stuff to try:

    * Upgrade to 1.7.2 which included fixes to the ipnhandler.
    * Make sure that *ipnhandler* is ignored by your caching plugins, etc.
    * Google Page Speed has been causing issues with this. If you are using it, stop using it and try another caching/etc solution (W3 Total Cache is good) that you can exclude files with.
    * If you are on lower tiered shared hosting, upgrade to a VPS or higher plan. The ipnhandler calls can sometimes timeout.
    * Consider switching to PayPal Express.
