Poll in Steps and Slider

I need to show the Poll questions step by step. When someone answered then another question will come so. And want to show this poll survey in the horizontal slider.

How can we do it? Please reply to me asap.

If possible please let me know also, I want the resulting graph will come after answering each question, not at the end of the poll or before the poll.

Now All question is coming together. So this process is not full fill my purpose.
