Poll wont work

Sorry to hear you are having this issue.

Indeed I see multiple console errors, including the Uncaught TypeError: $ is not a function

This means jQuery is being optimized and causing some issues, could you try to disable the WP Meteor and see if makes any difference?

If you already did, can you export both form and poll and share using pastebin or google drive?

I use your forms for about 7 months and so far I have never had a single problem.
The problem is also present when no plugin is active

Had you tested a different theme as well? the default WordPress theme

Could you also export the form, I added the Poll here https://patrickfreitas.net.br/form/ but it worked out of the box.
