Popup Disclaimer Form

I need a form that people can read and accept to proceed on my webiste. The popup I’m using doesn’t work properly, the form comes back up every time you go to the page, even in one visit (EMC2 Popup Form). Requests to the developer have not been answered.

Does anyone know a better plugin that can accomplish this task?
