Would you please check image is actually uploaded in media. you can check it in wp admin under Media->library.

We think it seems permission issue in uploads directory.

Yes image is actually uploaded in media…And The plugin is installed in a fully functional website with a lot of image uploaded.
The problem also occurs when I enter an image URL.

yes, I’m able to save setting and plugin work well, already tested with contact form inside. Only icon image function seems to have problems

This seems to be hosting security setting which does not allow to save url of the image. To confirm just remove http and domain name from the url of the image and then try to save plugin setting if it works then its hosting issue so please contact hosting guys.

I confirm… I’ve removed http and domain and now WORKS!

So if anyone will have the same problem and can’t contact the hosting, the solution is to upload icon on media library, copy the url, paste it and remove http and domain, eg. /wp-content/uploads/2014/09/xxxxxxxxx.png
