Post Pagination, setting a different meta title and description for each page?

I must apologise if this has been covered before – I’ve searched extensively but haven’t come up with a solution thus far, which is why I’m hoping somebody here will be able to help.

I have a huge post (14,000 words or thereabouts) which I’d like to break up into pages. The post contains several ‘parts’ for one of a better description, but as they’re all related to one another I want to avoid separate posts, and instead paginate a single post so that each part exists on its own page.

The problem is I’d like each page to have its own title and meta description, just like as if it were a separate post. So, for example, page one would be “my post part 1”, page could would be “my post part 2”, etc.

Is there a plugin that can achieve this? Ideally, the names of each part will also be displayed beneath the posts – so for example after reading part 1, the user can glance through the titles of the rest of the posts to decide which they wish to read. This is entirely optional

Failing this, is there a plugin that can link a given set of posts together as ‘parts’?
