Premium version charging percent instead of fixed

    I have purchased your premium version, but having a problem. (Tried to contact you via your contact form, but the form is broken)

    The 1st Additional fee is charging the correct amount ($2.95 fixed), but additional fixed options are charging a percent. Can you help?
