price-calc plugin

I’m having some trouble with the price calc plugin. The developer has dropped all support questions and left town. What I’m looking to do is create a dynamic form that adds additional questions as the user inputs his/her info.

It directly tells you how to do it in the directions but I’ve been working on it for 4 hours and still cant get it. I’ve tried every combo there is. Please help!
