Printable Online Property Rental Application Form?

Hi,
I would like to set up a system where by i might advertise a rental property in a paper as i do but with a proposed link to my WordPress site.

Till now i have had a two peace of paper application form i manually give. I’d like to duplicate that same system online so that it’s submitted and arrives in my email that i can print and file it away.

I like PDF’s because they seem robust and print well etc but this would not have to be the exact way. Can my existing application forms be scanned into something and converted somehow?

Any suggestions on what ways or steps to take and or any plugins that could manage this would be very helpful, thanks. The more simple the system the better!!
