different templates? or the same template? are the three hostings a different hoster or it is the same hoster with three domains/webspace?

that must be a “global” issue that the database table could not be written/filled. so it must be something that is on all your installations the same that causes the issue.

I have tested the plugin on several installations sincs WP 4.0 and everything works fine with installation/deinstallation and changing some options in the plugin.

try to add a line in the database table with phpMyAdmin with the values from my screenshot for example. And then try to change something in the settings page and proof if the changes are in the database table or if something is in the errorlog from WP

The changes i made in the settings page of the plugin are written and stored in the database. So in this way it should be fine and you can use the plugin.

You have to change the gravatar size to the size your template is using.

I found a little optical issue in the setting page when cUrl is the only one option to copy the images. I will fix it in the next update i will release this evening, i am working on the translations now.
