Problem: When user submits file for badge, it submits multiple random times

Hi BadgeOS, We’re kind of stuck and have tried everything we could to see what’s causing this problem but no luck so far. We wondering if you can provide us with some insight or at least some instructions on how to diagnose this issue.

Basically when a user submits a file attachment in order to qualify for a badge, the form gets submitted multiple times (seemingly random number of times, 3-6 times it sends duplicate submissions of the same submission) and then when the submission is completed the ‘thank you for submitting’ output and comment form is duplicated multiple times on the page. The process is shown in images below:

Once you click submit, the submission is sent MULTIPLE times for some reason (in this case, 3 times):

What could be causing these malfunctions?
