Problem with checkbox size in forms after updating template

In case you cannot see the image, the checkbox should be in the end of the form before the text ” Συναιν? με του? ?ρου? Χρ?ση?” where you can only see a line, this should be the checkbox.

Also in the following page, at the bottom of the page, the radio button when it’s not selected looks as a line too. There should be 3 radio buttons.
And under them, a check box and they all look like lines, something is happening with the width of the box.
