Problem with Jquery

Hi.
I’m having problems with Jquery on my blog. I installed a new form which isn’t working and their support said it was because “it appears that jQuery isn’t included on your page”
I run Thesis and went into the Thesis options and clicked to include Jquery but still no luck. I had a look at W3 Total Cache and couldn’t see anything wrong there (although I’m not exactly an expert).
When I view my blog in Internet Explorer it comes up with the following problems:
