BP Follow is a lightweight plugin that shouldn’t slow down your BP install.

Do you have any benchmarks proving that the plugin is causing your BuddyPress install to slow down?

The development version does have caching support and query optimizations:

You might want to try that out if your site uses an object cache plugin.
