I am seeing the same issue on my side where safari on mac and iPhone the multi step donation form does not display correctly but on other browsers it seems to be just fine.

@marcos1991 I would edit the description to issues with Safari browser (and possibly specify multi-step donation form unless you are having issues with the standard form as well)

Have you tried with a different device to discard this issue is not specific to your phone?
