Product order plugin (not ecommerce)

I’m trying to find a ‘product order’ plugin which behaves a bit like Excel, but with email functionality:

1. An admin creates/publishes a table/spreadsheet of products (with prices).
2. Customers fill out their details/quantities/etc, with automatic totals
3. Have a ‘send’ button at the bottom which sends an email of the order to the admin.
4. A print button would be nice too for oldschool users, but isn’t critical

I don’t mind paying, I actually prefer it for a bit of support. Is there a plugin out there which allows any of this? I’m not after an ecommerce solution.
