That said, publisher is becoming a frequently-requested feature, so I will look into adding it in next time I have time to work on the plugin.

You can categorize them however you want. However, if you want to have different lists to sort by, you’d need a new taxonomy for that. (Again, see that forum post linked above.)

Yes. Either yourdomain.com/book-review/ or any page you put the [book-reviews] shortcode on.
