Publishing doesn’t over-ride original post. Creates new permalink.

Hi there,

When I press publish, it states it is published, but it doesn’t override the original post.

Original post /my-eye-exam/
It creates a new post at /my-eye-exam/my-eye-exam/

I have searched the forums, and even though there is some similar topics, nothing has solutions for this.
