Pushover notifications not working

I think I have 2 issues, they may be connected.

https://www.dropbox.com/s/u21ukm0yd6hsi0v/pushover.png?dl=0My settings page when I select Pushover, reloads the page but still shows the email information boxes (from email, reply to, bcc, etc) that does NOT show on yours.
When I add my user key and save, no notifications are sent. I have tried with your default application and also with my own application key with the same result.
