Qualifying Criteria Form

I am looking for a plugin/ form where I can create a questionnaire for users to fill out to determine eligibility to see if they meet the qualifying criteria. The answers would be yes/no and ideally, I’d like to be able to have it give a response when the wrong answer is given that they aren’t eligible.
