Query Form

  • Hello,
    I’m hoping someone out there can help me. I’m working with a friend of mine to help them with their website. They want a form on their website that their clients can reserve a item. One of the fields they want is a drop off location as these items are delivered to different hotels. They wanted to create a query field that would be able to go out and pull the entire address information from a spreadsheet that would list all the different hotels they work with, there are about 200 hundred total. Is there a plugin that I could use to create this form with a query field? Or any other ideas?
