Question on directory/sub-directory creation

I have installed the plugi and it is cool.

I do have few questions:
1. How can user create his own Directory and Sub directories? In my case user can create multiple albums (directories under his user name and sub directories under each Album for sections).
2. How can I include form elements? I do need to collect some user inputs such as Album Name, Custom selections.

Any help is much appreciated.
