Questions about Email Capture on Pro Edition

I have a couple of questions regarding the Email Capture feature on the Pro edition…

1) Will someone is downloading multiple items, will it ask for the email address for every item or just the first time?

2) Is it possible to ask for more information in addition to the email address (name, city, company, etc.)?
