quiz email confusion

I set up the email to send me a response for every user of my quiz. almost everything seems to work good, except in the body of the email I want the username and email.
Several issues:
1) There is no place to put an explanation of why I ask them to fill out their name and email in the summary, before results. I am using a text box for the explanation, but that creates an extra fill-in box.2) In the return email to me, I use the var $username, but there is no var for the user email, even though there is an email category in setting up the quiz.
3) On the return email, instead of getting the user name, I get the IP. I trid getting both username, and IP, but then I just get IP twice.
I would very much appreciate understanding how to get around these issues.
