    There’s lots of forms plugins out there, actually. I have never used the Calculated Fields one and I’m totally not trying to knock it, but you should recognize that it hasn’t been out long. I try to look for plugins that have a longer track record before using them for any business applications. Gravity Forms and Formidable are two that have been around for a long time and are proven solutions; either could definitely handle this requirement.
