Re-ordered list keeps posts removed from the categories

We are using your product to arrange posts by category. A problem we found is that if we change a post out of a category into another one or remove a category from a post when we go to arrange posts in that category the post still shows up. I have tried clearing the cache with no luck. I disabled all plugins but yours and switched to theme Twentytwelve, no luck. I read another post in your support forum with a similar problem. I saw no resolve there. I wonder if you have explored this issue any further and found the problem. Any insight would be helpful. I can find no other plugin that performs this task so I hope we can find resolve. Thanks
