Recommendations?: Form with conditional questions, math functions, sliders?

  • Need a plugin suggestion: I have been asked to create a form with these functions:
    1. Which picture do you like? Click one.
    2. How many people will be joining you? (1-7 pull down or click on image)
    3. Cost per person: (Package 1: $XXX/the answer to question 2; Package 2: $XXXX/by the answer to question 2, etc. as a Pull down menu.)
    4. Where do you see yourself on this spectrum? (Slider)

    It would be great if the survey form had a lot of eye candy built in. Suggestions?
