    Verified as working on my dev site — so something on your site is breaking this. Could be a JS error, could be in the theme or a plugin.

    Can you:

    Send me a link to a form and what the thanks URL should be so I can look into it?

    Switch to a bundled theme (e.e. 2012) and see if that fixes it?

    Disable all other plugins and see if that fixes it (at which point, you can re-enable them one at a time until it returns to find the conflict).
