Redirections Fail After Period of Time

I have a peculiar issue that is reoccurring.

I will create a redirection and it will work for a period of time (around 3-5 days) and then return a 404.

If I recreate the redirection, it will work again.
If I moved the redirection from the current group it’s in to another group it will work again.

This is happening both with redirects with and without regular expressions.

Has anyone had a similar experience or insight?
