    If you post a link to your page with that shortcode I might be able to figure out what is going on. That shortcode should definitely work. It seems like either the checkout page is requiring membership for some reason (don’t check levels on the check page) or the level being passed by the form is disappearing and the checkout page is redirecting back to the levels page to choose a valid level. This might happen also if level 4 doesn’t exist or is hidden/blocked from new signups.
