Sorry for the late reply. I am not getting email notifications when people post in this support area. I’m sure it’s a setting somewhere in my WP profile that I can update so I’ll need to check into that.
Yes, the plugin in current form only shows 3 default fields. As mentioned above, we had 4 updates planned for the Premium version by end of year. We have completed 2 of these (email notification, and drop down list) and the next one that is currently under development is the ability to add more fields to the form. This particular feature will be included in both the free and Premium version.
The goal was to have all 4 updates done by the end of the year and we’re currently on track to be somewhere in the vicinity of meeting that timeline goal.
