New registrations are being automatically approved when they shouldn’t be!

Since the last update we have seen examples of new users registering to use the site being granted automatic access – despite the settings being set to push all new registrations to be admin approved.
What is more confusing is this is not all new registrations – but no obvious consistency with those that are getting through the net.
We have rolled back to an earlier version for now, but require this be addressed as quickly as possible.
