Registration Form Reloads with no submission

Do you have an Email Address field to each Register form?

I have Email field on the other two forms, not in this particular form.
I tried to remove the fields on this form to find out if theres any problem with the fields, but even if I place one field(username), the form is not working with no errors.
ps: I also turned on WordPress debug mode but still no errors!
