Removing Main Payment Options

I am trying to get just the “other payment” option to show. Unfortunately, even if I leave the main preset options blank, it creates a drop down arrow with nothing there.

You have a screen shot that clearly shows just a reference field and a set your own amount box. I am trying to get that. Please advise on how I remove the preset amount drop down.
