    Thanks for reaching out to us. I’m sorry there was an issue with the fields.

    Did this happen when you originally installed the plugin or did this happen after you made any specific changes? I wasn’t able to replicate this on our test sites.

    In the meantime, you can delete these duplicated fields by going to Directory > Directory Content > Form Fields.
