Repeating footnote numbers

Pasting up my first page and ran into a problem.
The footnote numbers fail to increment properly in text. In my case the number 8 repeats once. This also results in the end-of-page footnotes being one shy of the correct footnotes (e.g., there should be eleven footnotes and there are only 10).
