Reply option for unregistered users

I can set up a page with the reply at the bottom of the page but it only seems to show up for admin. I would like users to be able to leave me a note even if they are not a registered user or if they are not logged in.

Although it is to be set up so that any comment will need my permission to have it posted, these comments are intended to be directed to me only and will never be posted anyway.
