Request Form Plugin?

Hi Everyone,

I have had a search in the plugin section but can’t find what I need.

Do any of you know of a plugin available which will be suitable for a brochure request purpose?

Ideally it would provide: A form with name, email address etc which, once filled in and sent, would add the persons details to the back-end database so that this could be accessed and then brochures sent out.
