Required fields are not respected – Using Ninja Forms

We are using Ninja forms for our applications and we have multiple fields marked as required. The issue is when the fields are not filled out and the form is submitted, the page reloads as if the application was successful and the application is collapsed but there is no indication that the form was actually not sent and some fields were not filled out correctly.

How do we get an alert stating that required fields were not completed?

Please advise.
