Required Fields/ Conditional Fields Not Working

Just an FYI for everybody.
After installing Form Maker today I had trouble getting Conditional Fields to work. Also, marking a field as Required did nothing. I had trouble getting conditional logic to work on another form plugin as well.

I deactivated all of my plugins and reactivated one at a time. The SG Optimizer plugin was the culprit. More specifically, Remove Query Strings From Static Resources was causing all the problems.
