Required Product Input Field module not always showing on order

We have the Product Input Field module added to all of the books in the Shop. It’s a required field so it needs to be filled out. It’s placed before the add to cart button and looks like:
“Book Customization
Who would you like your book made out to? ”For the past couple of months, this field is hit or miss if it’ll show up on the order that is submitted, even though it’s a required field.
