Reset password 404 page

My client reset her password while logged in and then forgot it. When she went to log in again, she clicked on the “forgot password link” and it takes you to a 404 page:

https://www.vicariouslysf.com/lost-password/

Does anyone know how to fix this without being logged in? I dont know if it has anything to do with plugins, but i know she doesnt have very many. Just askimet and google dashboard.

Any advice would be great.
