category restriction is introduced in version 1.8.5. you should update your copy.

1 – But how can I select several membership level ?
For example give the access at the Category A for the users level 1 and users level 2 ?

2 – When I select a level and I update, I don’t see each level I have choosen ?

its a two step process:

step 1 (protection): select “General Protection” in the dropdown and check the categories you want to protect. then press “Update” button.
this will enable protection to selected categories.
step 2 (permission): in this step you will allow certain membership level to view protected categories. select select any membership level except “General Protection”. check the *protected* categories that you want this membership level have access to. then press “Update”.

If I give the access at Category A for the level 1, can I restrict a post into the Category A for the level 1 user ?

category restriction has higher precedence. if you restrict category then all posts under than category is restricted.
