Rewrite URLs for items in portfolio_category

Hi Devin,

Thank you a lot for awesome plugin! This is exactly what I was looking for to display my experience.

I figured out how to change portfolio slug to the other name (‘/cases’), but couldn’t find out how to make portfolio more structured.

Right now the items in my portfolio look like this:
mysite.com/cases – for portfolio page
mysite.com/cases/item1 – for single item page
mysite.com/portfolio-category – for category

So, the question is how to make the category pages looks like
mysite.com/cases/[portfolio-category]

Also would like it works for category-tags

And, if possible, change URLs for single items like that
mysite.com/cases/[portfolio-category]/item1

I understand, that last one requires the item should have the only one category.
