Risk assessment questionaire, output individual graphic results

Before I bid on an RFP, I want to determine if formidable forms can handle the following survey requirements related to alcohol use to determine if the person is at risk.
Survey is in 2 parts – a background /demographic survey and a current alcohol use survey.
Each survey is divided into sections where each section contains one or more questions about at what age, how much and how frequent alcohol is consumed. Scoring is by section…they get a point if their answer of “how frequent or how much, etc is over a certain threshold.

At the end of the survey, if the total of all section scores is over a certain threshold, they get a “warning, you are at risk” page or a “you are ok” page.

Next step is that a saliva collection kit is mailed to each participant that completes the surveys. The date that the saliva kit was sent to the participant needs to be manually added to the survey results data (i.e. formidable data base). Once the saliva samples are received back and processed by the lab, the genetic data obtained from the saliva kit also needs to be added formidable forms database. Can a wordpress administrative user manually update the survery results database with both the date and the genetic information?

Finally, an overall risk score is calculated from the survey results data and the genetic data. That overall score also has to be added to the formidable forms survey data and the participant is notified to log back into the website to view a graphical representation of their overall risk score. What type of graphical tools / icons would be available in Formidable Forms?
