Same form in two pages: one is working, the other one is not

I have two pages where the same form is embedded.
The form use your plugin for multiple columns.In the first page, everything is working correctly:

(to see the form, please scroll down and clic on the red button labelled “Conferimento mandato”).
In the second page, the form is ignoring the multicolumn layout (only from a certain point ahead):
https://www.circolobdr.it/offerta/isola-delba-capoliveri-uappala-hotel-lacona-2/
It’s seems that needed css file (https://www.circolobdr.it/wp-content/plugins/gf-form-multicolumn/includes/public/css/gf-form-multicolumn.css) is included in the first page but not in the second.
Can anybody help?
