Searching for a simple form with paypal at the end of the form

I want to create a simple online registration form for people to register for my courses. It needs to have a paypal button at the end to complete the registration.

I looked at “Events Registration” which is quite indepth and powerful, but it’s much more than i need.

Any suggestions?
