Second Form Needed

Love your plugin!!

Is it possible to have another form to use on a different page? I currently have the plugin in use on the Contact page; however, there’s a Support Committee page which requires a form as well.

Is there any way to go about duplicating the form, but sending it to a different email for the Support Committee page?
