    Du you have any other plugins installed that do some changes with user profile, user name, …?
    I’m a little bit confused because you are the only one reporting this problem.
    So there must be a conflict with another plugin.
    Please deactivate all other plugins (do this at a copy of your site and not at your productive environment) and try if the error still appears after every deactivation.

    This is hardly a feasible approach, as we run at least 30 other plugins, to trouble-shoot this one. We reported the very same bug before (several months ago).

    We will just have to omit the use of your plug-in … too bad.

    Don’t you have a testing environment next to your productive site where you test new plugins, updates and so on?

    no we don’t

    the site is so busy with 230,000-plus users that it is hard to maintain a working copy

    You really should think about this.
    The number of users is not a problem. I also do this for my site, I wrote a little shellscript that rsyncs the web root to a testing subdomain, dumps the database and replaces all urls with the new testing subdomain url and anonymizes user data and user email addresses. It just needs one script, one command line in the shell, to get a working copy and it just takes 2-5 minutes.

    I’m sorry that I cannot help you, the effort trying to find the error that appears only at your site is a multiple than deactivating modules and testing what module makes the troubles… I hope you can understand this. If you can find out what modules causes my plugin not to work correctly I’ll try to fix your issue.
