Seems to over-write .htaccess on every page view/once a minute

I would take a look at the logic in this plugin for how often it is reading/writing to the .htaccess file. I noticed that with the plugin active, everytime my page was loaded my .htaccess was being updated with the timestamp of the page load.

This would be okay for short periods if it always put back what I had in the .htaccess, but I noticed it causing errors like incomplete files/empty .htaccess.

Hard to recommend this plugin until this is fixed, especially if you have a site that has many pageviews per minute.
